New Version of Forms on Disk

We are pleased to announce that the latest version of Forms on Disk (4.5.0), featuring additional functionality and improved features, is due for release shortly.

This upgrade will be sent to all existing Forms on Disk customers at no additional cost. Please look out for your new installation CD and information pack arriving in the post over the next few weeks.

The above forms have been amended as per guidance from HM Revenue & Customs.

CIC14 - Application to Alter the Objects of a Community Interest Company

CIC34S - Community Interest Company Report (Simplified)
CIC34D - Community Interest Company Report (Detailed)
CIC36 - Declarations on Formation of a Community Interest Company
CIC37 - Declarations on Conversion to a Community Interest Company
CIC36-37-X - Continuation Sheet (for use with CIC36 and CIC37)
CIC53 - Declaration on Dissolution of a Community Interest Company
CIC53-X - Continuation Sheet (for use with CIC53)

All of the above new forms are for use by community interest companies. Guidance has been taken from ‘The Community Interest Company Regulations 2005’ and the ‘Regulator of Community Interest Companies.’ Further information can be found at http://www.cicregulator.gov.uk/index.shtml

Amended Legal Aid Forms
CDS6 - Contract Work Report Form
CDSC-GN - Guidance for Reporting Work on Form CDS6 & CDS7

These forms have been amended as per the Legal Services Commission (LSC), who have asked that the reporting of Criminal costs now be VAT exclusive. The LSC say that this will bring Crime Reporting into line with Civil Reporting and allow for circumstances where the addition of VAT is not appropriate.

Therefore, form CDS6 has been amended to add two new columns pertaining to VAT, and form CDSC-GN has been amended to add a new section 2 pertaining to VAT. The former section 3 is now section 5. This document has also been amended throughout to reflect the VAT requirements and online submission as, from 1st August 2008, CDS6 submissions should be made electronically.

Form CA31 Amended
CA31 - Stock Transfer by Company

Page 1 of the form remains the same but because the current certificate is no longer required, it will be removed and replaced with a new certificate for transfers on sale where the consideration is £1,000 or less. This will bring the form in line with CA30.

Form N244 Amended

N244 -Application Notice

This form was amended as per HMCS/MOJ. The form as amended does not depart in substance from the old version, but the content and layout has been redesigned to improve usability. Additionally, there is a new page 3 which contains guidance notes.

Updated Divorce, Family & Probate Forms

The following forms in Divorce, Family & Probate have been amended on page 2 to add space in the signature field.

PR1 - Oath for Executors - no settled land
PR2 - Oath for Administrators - no settled land
PR3 - Oath for Administrators - with Will annexed
PR4 - Oath for Administrators - de bonis non
PR5 - Oath for Administrators - with Will, de bonis non
PR8 - Oath of Attorney of an Executor

Advance Information - Amendment to Form CIT
Application in connection with court proceedings, insolvency and tax liability

Reason:

Amendments to certificates H, I, M and O as per SI 2008/574, which abolishes the Assets Recovery Agency and transfers its functions to several other organisations.

This form is now available via online update facility.


Advance Information - Amendments to Legal Court Forms
 
As per Her Majesty's Court Service, the Civil Procedure Rule Committee has agreed that changes should be made to both N150 and N149 to reinforce the Alternative Dispute Resolution (ADR) message. N151 form has been changed to bring it into line with the N150.

These forms are available via the online update facility now. Their effective date is 1st April 2008.

A number of the MasterPack forms are being updated by the Legal Services Commission. Most of these come into effect 1st October with the remaining coming into effect 1st November.

From 1st August there will be two new forms replacing the existing CON29, as below.

The Law Society have this morning issued us with the new forms, both of which are now ready to download via the Forms on Disk Online Update. Using the Online Update facility is the quickest and easiest way to ensure that your forms are kept up-to-date and that you have the correct versions in time for the 1st August changeover date.

New Gambling Act Forms Offer 27/04/07
 
Gambling Act 2005 & Forms on Disk
The Gambling Act is scheduled to be introduced on 1st September this year. In view of this, Shaws are in the process of preparing a complete set of relevant forms for use by local authorities. We are aware that DCMS will be supplying the prescribed forms and that LACORS are also working on a small number of templates. Shaws will, where possible, use these templates and recreate them in our Forms on Disk package, in addition to a number of other non-prescribed forms that will be required.
 
The aspects of the Act for which forms are in preparation are as follows:–
  Part 8 - Premises Licences
Part 9 - Temporary Use of Premises
Part 12 - Clubs, Pubs, Fairs & Commercial Clubs
Part 15 - Inspection
Schedule 9 - Applications for Casino Premises Licences
Schedule 10 - Family Entertainment Centre Gaming Machine Permits
Schedule 11 - Exempt Lotteries
Schedule 12 - Club Gaming Permits and Machine Permits
Schedule 13 - Licensed Premises Gaming Machine Permits
Schedule 14 - Prize Gaming Permits
 
 
The cost of a licence – enabling full use of the above forms – will be £1,000. However, we are offering a 40% early bird discount on any orders placed by 31st August 2007. If you work collectively with other local authorities, and are considering implementing this Act jointly, we will also consider further ‘group’ discounts based on multiple purchases.

Power of Attorney Forms 18/04/07
 
Enduring Power of Attorney:
In December 2005, the Public Guardianship Office changed the forms for making an enduring power of attorney to include references to civil partnerships. It also changed the form of application to register an enduring power of attorney.
 
In December we amended our form to account for the above changes and an update disk was to all relevant customers. This form came into effect 1st April 2007. Providing all updates have been installed, our version of the form is correct, and is now valid until 1st October 2007.
 
Lasting Power of Attorney:
Further to several recent customer enquiries regarding the Lasting Power of Attorney, below is a response we received from the Public Guardianship Office:
 
"The forms for the new LPA instruments (Health & welfare and Property & Affairs) will shortly be laid before Parliament. This process will take 40 days so we anticipate the new forms will be available towards the end of July. We will also be issuing with the forms guidance for the donor, attorney(s) and the certificate provider.
 
There will also be a new application from for those applying to register and a new form used to notify the donors named individuals. You will be able to make LPA's from October 2007.
 
Enduring Powers of Attorney can still be made up to end September 2007 and registered after that date.'
